What Is an IPOP Prosthesis?

Immediate Post-Operative Prostheses (IPOPs) are temporary prosthetic limbs fitted directly after amputation surgery, either in the operating room or within the first few days of recovery. They serve as an early support system for the healing limb and protect against trauma to help manage pain and swelling.

Traditionally, patients wait until their surgical wounds have healed before receiving their first prosthesis, which can take anywhere from three to twelve weeks. During this waiting period, they rely on wheelchairs, crutches, or walkers for mobility, which can lead to complications like muscle weakness, joint stiffness, and even injury from falls while moving around on one leg. IPOP allows patients to start moving and bearing weight on the affected limb much sooner, reducing these risks and allowing for a safer and more controlled start to rehabilitation.

IPOP Prosthesis

Originally developed in the late 1950s, IPOP use has shown significant improvements in rehabilitation outcomes. Initially, IPOPs weren’t easy to remove, but in 1995, the device was redesigned to allow for easier removal at the request of surgeons. Today, IPOPs are complete prosthetic systems, including a socket and necessary components, designed to stabilize the limb and support early rehabilitation efforts during the critical healing phase.

Types of Immediate Post-Operative Prostheses

Post-op prostheses (IPOPs) are available in two primary types: hand-molded and prefabricated. Each type offers unique features and benefits so they’re ideal for different patient needs during recovery.

  • hand-molded IPOPs

    Hand-Molded IPOPs

    • Traditional IPOP: Made from plaster bandages, these sockets are custom-molded directly onto the patient’s limb. They provide controlled partial weight-bearing by attaching a foot and pylon to the plaster socket. However, they are non-adjustable and non-removable, meaning the entire cast must be cut off and reapplied if the residual limb needs to be examined or if the limb size changes.
    • Removable Rigid Dressing (RRD): This variation is also made from plaster but is removable so it allows for minor adjustments using socks to accommodate limb changes. While it offers easier access for inspection and care, the RRD is non-weight-bearing and still requires reconstruction as the limb heals and changes shape.
    • Tarsal Tunnel Syndrome: Tarsal Tunnel Syndrome occurs when the nerves passing through the tarsal tunnel near the ankle joint become compressed. This can cause pain along the foot, including under the heel, leading to discomfort during movement.

    Prefabricated IPOPs
    Prefabricated IPOPs are made from plastic and come in standard sizes. They are:

    • Adjustable and Removable: These IPOPs can be easily adjusted to accommodate changes in the residual limb size and shape, making them more convenient for ongoing assessments and modifications. They can also be removed to allow the healthcare team to monitor healing or address any complications.
    • Convenient for Monitoring: Their removability facilitates daily evaluations of the limb to allow for a more responsive approach to rehabilitation.
    • Limitations: Prefabricated IPOPs may not fit well on very long or irregularly shaped limbs and are generally more expensive than hand-molded options.

Who Can Benefit from IPOP Prosthetics?

Not all patients are ideal candidates for immediate postoperative prostheses, and it’s up to the physician and rehabilitation team to determine if the benefits outweigh the risks. With attentive care from a skilled rehab team, it’s easier to avoid or minimize potential complications. It's crucial for patients using an IPOP to carefully control the amount of weight placed on it, as putting too much pressure on the wound too soon can cause damage. Additionally, complications can arise with or without an IPOP because of underlying conditions, such as infected or non-healing surgical wounds. In these cases, IPOP use may be temporarily stopped until the issue is resolved.

  • Immediate Application

  • Early Use and Adaptation

    - The IPOP helps the patient to start gentle weight-bearing activities almost immediately, promoting circulation and reducing the risk of complications.
    - The prosthetic includes a pylon (support rod) and foot section attached to the dressing to allow for limited walking under supervision.
    - This stage is crucial for the patient's psychological adjustment, as it offers the experience of ambulation, or the ability to walk independently, soon after surgery.

  • Monitoring and Adjustments

    - Regular follow-ups are needed to monitor the healing of the residual limb (stump) and the fit of the IPOP.
    - Adjustments are made as needed to accommodate changes in swelling and limb shape.

  • Transition to a Temporary Prosthesis

    - As the residual limb heals and stabilizes in shape, usually within a few weeks to a few months, the patient transitions from the IPOP to a removable rigid dressing or a temporary prosthesis.
    - This temporary prosthesis is designed for further functional training and to facilitate continued adaptation to using the prosthetic.

  • Fitting the Permanent Prosthesis

    - Once the residual limb has fully healed and reached a stable size and shape, a process that typically takes several months, a permanent prosthesis is fitted.
    - The permanent prosthesis is custom-made to provide the best fit and function based on the patient’s needs and lifestyle.
    - This prosthesis may be adjusted over time to ensure optimal comfort and function.
